Initial Consultation + Possible Exam and Treatment

Dr. Sundy makes sure all of his patients feel welcome, safe, and taken care of. During your first visit, Dr. Sundy will review your medical history and check your body’s mobility comparing the ranges of motion of most joint of the body. Dr. Sundy carefully listens to your needs and concerns, and creates an individualized treatment plan. Whether your goal is to reduce pain or to improve your range of motion, Dr. Sundy is here to help you to achieve your heath goals in a way that is educational and interesting, painless as possible, entertaining at times and relaxing at others and occasionally described as an “out of body” experience .

For a shorter time in the waiting room, please fill out the New Patient Form and bring it with you.

 

Please allow 90 minutes for your complimentary initial 15-minute consultation with Dr. David Sundy in case you would like to continue with an exam and treatment. A complimentary 15-minute initial consultation is required for first time patients as well as patients who have not been seen for 90 or more days.

The initial consultation consists of a 15-minute chat with the doctor to be sure he is the most appropriate professional to assist with your needs. If he believes you would be better served by another professional in the community then he will refer you to that particular practitioner. If he believes he is the most appropriate doctor for your condition, then you may continue with exam and treatment, unless your condition requires outside lab testing or additional information.

Active Healing-SF is committed to making chiropractic care more accessible for patients. Active Healing-SF offers a substantial discount for low income, i.e. those earning less than $83,000 per year, full-time students (with Student ID) and patients age 65 and up.

Payment must be rendered upon scheduling or at the conclusion of each office visit. A super bill can be provided for the purpose of insurance reimbursement.
